Comparison of Multilevel with Single Level Injection during Lumbar Sympathetic Ganglion Block: Efficacy of Sympatholysis and Incidence of Psoas Muscle Injection

چکیده
BACKGROUND We prospectively evaluated the incidence and possible factors causing intramuscular injection during lumbar sympathetic ganglion block and compared the multiple needle technique to the single technique to obtain a profound and complete block effect. METHODS Among 83 patients, 58 patients (group A, n = 27, multiple needle technique and group B, n = 31, single needle technique) were reevaluated for the changes of skin temperature (Ts) and mean segment of longitudinal contrast spread. After injecting the contrast agent, the incidence of psoas muscle injection and the change of Ts was compared between two groups. RESULTS The incidence of psoas muscle injection was 21.3% (46/216) and it was associated with the level of injection (L2) significantly (chi(2) = 14.773, P = 0.001). DT(post) (postblock temperature difference between ipsilateral and contralateral great toe, 4.6 +/- 2.8, 1.8 +/- 1.6, P < 0.001 for group A and B) and DT(net) (DT(post) - DT(pre), 3.9 +/- 2.7, 1.5 +/- 1.5, P < 0.001 for group A and B) was significantly higher in group A. The mean segment of longitudinal contrast spread was 8.1 +/- 0.9 for group A and 3.2 +/- 1.6 for group B (P < 0.001). CONCLUSIONS The LSGB at the L2 level showed the lowest incidence of psoas muscle injection of contrast. Multiple needle approach showed more significant increase of DT(net) and DT(post).

Lumbar Sympathetic Block for resistant to treatment causalgia in patient with opioid dependence.
Abstract Background & Aim: Lumbar sympathetic block is a commonly used technique for sympathetically mediated pain in lower extrimity. Causalgia (CRPS type 2 ) is also accepted to be associated with sympathetic system activation. Treatment of this condition is difficult, especially when the patient has opioid dependency. there has not been any report regarding lower-extremity lumbar sympathet...

متن کاملOnset of action and duration of efficacy of inferior alveolar nerve block versus single lingual subperi-osteal injection of 4% articaine in mandibular second molars: A randomized clinical trial
Background and Aim: Achieving adequate pulpal anesthesia could be challenging in mandibular molars. There are some disagreements about the success rate of local infiltration anesthesia with articaine as primary injection. Therefore, the aim of this study was to assess the efficacy of 4% articaine lingual subperiosteal injection as the primary injection for permanent mandibular second molars in ...
متن کاملPsoas compartment block
Psoas compartment block (PCB) is a peripheral regional anaesthetic technique that blocks the main components of the lumbar plexus, namely the femoral, lateral femoral cutaneous (LFC), and obturator nerves as they run within the psoas major muscle. The psoas compartment block is also known as the posterior lumbar plexus block. A posterior approach to the lumbar plexus was first described by Winn...

Anesthetizing the lumbar plexus at its origin facilitates a more "complete" psoas compartment block compared to peripheral approaches. It is usually performed using surface anatomical landmarks, and the site for local anesthetic injection is confirmed by observing quadriceps muscle contraction to peripheral nerve stimulation. Ultrasound may provide guidance alone or together with the aid of ner...
